Registration Waivers

What was all that "fine print" I agreed to when registering my child? You will find all the waivers listed below so you can understand them in detail.

Medical Release Waiver (You will complete and sign this form at tryouts)

Recognizing the possibility of injury or illness, and in consideration for US Youth Soccer and members of US Youth Soccer accepting my son/daughter as a player in the soccer programs and activities of US Youth Soccer and its members (the "Programs"), I consent to my son/daughter participating in the Programs. 
Further, I hereby release, discharge, and otherwise indemnify US Youth Soccer, its member organizations and sponsors, their employees, associated personnel, and volunteers, including the owner of fields and facilities utilized for the Programs, against any claim by or on behalf of my player son/daughter as a result of my son's/daughter’s participation in the Programs and/or being transported to or from the Programs. 
I hereby authorize the transportation of my son/daughter to or from the Programs. 
My player son/daughter has received a physical examination by a licensed medical doctor and has been found physically capable of participating in the sport of soccer. I have provided written notice, which is submitted in conjunction with this release and attached hereto, setting forth any specific issue, condition, or ailment, in addition to what is specified above, that my child has or that may impact my child's participation in the Programs. 
I give my consent to have an athletic trainer and/or licensed medical doctor or dentist provide my son/daughter with medical assistance and/or treatment and agree to be financially responsible for the reasonable cost of any such assistance and/or treatment.

Photo/Video Release (You electronically sign this when you register with our club)

I hereby grant MACOMB SOCCER CLUB (hereby known as MSC) permission to use the likeness of my picture (including any with team players, team officials, and team parents) in a photograph and/or video in any and all of its publications, including website entries and any other media form without payment of any other consideration. 

I understand and agree that these materials will become the property of the MSC and will not be returned. I hereby irrevocably authorize MSC to edit, alter, copy, exhibit, publish or distribute photos or videos for purposes of publicizing the MSC or for any lawful purpose. 
In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my child's pictures likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph or video. 
I hereby hold harmless and release and forever discharge MSC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. On behalf of my child, I have read this release before electronically signing this registration form and I fully understand the contents, meaning and impact of this release.

Accident Waiver and Liability (You electronically sign this waiver when you enroll in tryouts)

Accident Waiver and Liability form- I acknowledge that participation in the MACOMB SOCCER CLUB (hereby known as MSC) programs carries with it the potential for personal injury, up to and including death. The risks include, but are not limited to, those caused by the terrain, facilities, temperature, weather (including lightening), equipment, vehicular traffic, and actions of other people, including but not limited to other players, coaches, officials, volunteers and spectators. 
I hereby assume all risks of participation in the MSC programs. I certify that I am physically fit, and have not been advised otherwise by a qualified medical person. 
I acknowledge that this Accident waiver and Release of Liability form will be used by the MSC, and that it will govern my actions and responsibilities while participating in the MSC programs. In consideration for my participation in the MSC, I hereby take action for myself my executors, administrators, heirs, next of kin, successor and assigns to: 
1. Waive, release and discharge from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me, including as to my travel to and from MSC events, the MSC, its elected and appointed officials, employees, volunteers, representatives, assigns and/or others working on behalf of the MACOMB SOCCER CLUB, and 
2. To the fullest extent permitted by law, indemnify and hold harmless the MSC, its elected and appointed officials, employees, volunteers, representatives, assigns and/or others working on behalf of the MSC from any and all liabilities or claims made by other individuals or entities as a result of or relating to my participation in the MSC programs. 

I hereby certify that I have read this document and understand and agree to its content. 

Parent-Guardian Waiver I am the parent, natural guardian, or legal guardian of the minor named above. I am acting in that capacity and agree to the fullest extent permitted by law to save, hold harmless and indemnify the VARDAR EAST SOCCER CLUB, its elected and appointed officials, employees, volunteers, representatives, assigns and/or others working on behalf of the VE, its elected and appointed officials, employees, volunteers, representatives, assigns and/or others working on behalf of the VE arising out of the minor's participation in the VE. 
I further release the VARDAR EAST SOCCER CLUB, its elected and appointed officials, employees, volunteers, representatives, assigns and/or others working on behalf of the VE on behalf of the minor, the parents, or the legal guardian as well.  I understand my electronic signature states that I have acknowledged and agreed to the Accident Waiver and Liability statement.

Code of Conduct (You will be electronically asked to agree to these codes when you accept a roster spot on a team)

We will refrain from booing or yelling at officials at any time during a game because 
1. It sets a poor example of sportsmanship, 
2. Reflects negatively on our programs, our community, our team, our players, and ourselves. 
3. Most officials do the best job they can with limited training.
4. The official seldom determines the outcome of the games. 

We will refrain from yelling at players on either team because 
1. They are young players, prone to mistakes due to lack of experience 
2. Our attempts to help may only confuse the player. The coach is best equipped to analyze and correct deficiencies. 
3. We will treat all players with courtesy, respect and consideration. 

We will refrain from being argumentative or using abusive language towards supporters of the opposing team because 
1. Others judge us by our words and actions. Sportsmanship should be the verdict. 
2. We will conduct ourselves in such a courteous and restrained manner that if called upon to do so, we could line up and shake hands with each supporter of the opposing team. 

If our team loses, we will demonstrate our ability to cope with the loss in both deed and word because 
1. In athletics, as in other aspects of life, it is not always possible to win no matter how great the effort. 
2. When victory eludes us, we must learn to accept defeat graciously as we do our triumphs 
3. It may be possible that a loss is due to the fact that the opposing team played the game better than our team 
4. Our players should learn from our reactions to a loss. 

The real purpose of youth soccer competition is to have fun. Improve skills. Learn sportsmanship, develop a sense of responsibility and self-discipline, develop group loyalty, learn to compete within established rules, accept decisions of authorized officials, and to develop sound minds and bodies. I/we have read and understand the above statements. I/we agree with these statements and will adhere to the code of conduct set forth by the MACOMB SOCCER CLUB in the spirit of sportsmanship, teamwork, and benefit to all. Parent/Guardian when signing electronically you are acknowledging that you have reviewed this with your minor child.

Registration Agreement

I acknowledge I have read and understand the Code of Conduct, Accident Waiver and Liability, Parental Authorization for use of Photographic Images and the Concussion Waiver form.  I have reviewed all forms with my child and discussed every form with them.  All forms can be found in the club policies section of website.

Electronic Payment Agreement

I understand that all payments will be automatically deducted by the credit/debit card submitted or electronic checking account on file. Return check fee is $25.00.  If enrolled into a payment plan, all payments must be paid by due date or a $25.00 fee will be added to each payment that is not paid on time.

Late Fees 
A late fee of $30 per calendar month will also be applied to accounts that are outstanding after the 1st of each month.
